Step 8: Give School Questionnaire Login Form and the School Questionnaire Manual to Principal
The school contact will receive the School Questionnaire Login Form with the assessment materials. The School Questionnaire Manual will also be provided.
Print and give the School Questionnaire Login Form and the School Questionnaire Manual to the school principal as soon as the assessment materials arrive. If a school does not have a school principal, then the school contact must identify another appropriate person to complete the School Questionnaire. Inform the school principal or designate that the questionnaire takes approximately 60 minutes to complete. It may be completed anytime but must be submitted no later than May 30, 2025.
The School Questionnaire Login Form includes login credentials for accessing the School Questionnaire as well as the link to the School Questionnaire (also available here).
The School Questionnaire Manual provides guidance on answering the different types of questions and how to troubleshoot technical issues.
Additionally, the manual includes a set of questions that also appears in the School Questionnaire. Before completing the School Questionnaire, the principal or designate should review these questions and gather the relevant data to answer them.
Please note: These answers must be entered online into the School Questionnaire and not the manual itself.
