List of Students

  • Student name: Type the full name (first and last name) of each student enrolled in the class.

  • Provincial Student Number: Enter the provincial student number assigned to the student

  • Date of birth: Enter the date of birth (DD/MM/YYYY) of each student enrolled in the class.

  • Sex: Enter the sex of each student listed as follows: F=Female; M=Male; O=Other

  • Special education needs (SEN) code: Input one of the three following codes to identify students with special education needs:
    • 1= Students with functional disabilities
    • 2= Students with intellectual disabilities and/or socio-emotional conditions
    • 3= Students with limited assessment-language experience

  • Alternative formats: Alternative formats can be provided for selected students if, and only if, the students receive such accommodations in an assessment situation during their regular school program. To request one of the alternative formats, record the corresponding code on the Student Identification Form for each student requiring an alternative format:
    • Code A – Audio
    • Code C – Coloured background*
    • Code D – Dyslexia font
    • Code L – Large print
    *Note: For coloured background, please indicate the background colour required (e.g. blue).

  • Language of assessment: The language of the assessment should be the same as the language of science instruction. If the default language is incorrect, select the correct language of assessment for each student.

  • Teacher information: Enter the name of each student’s science teacher for the 2022–23 school year in the respective column. If a student is not currently enrolled in a science class due to a semester schedule, please include the name of the teacher who taught or will teach them science in the 2022–23 school year.
