Step 6: Distribute Questionnaire Login Forms
The school contact will receive the following questionnaire-related forms with the assessment materials:
- School Questionnaire Login Form: For the school principal or designated staff
- Teacher Questionnaire Login Form: For the teacher(s) listed on the School and Teacher Questionnaire Tracking Form
- School and Teacher Questionnaire Tracking Form: For the school contact to track school and teacher questionnaire completion
