Step 6: Receive Assessment Materials

At least five days prior to the first assessment date, the school contact will receive one email containing the electronic assessment forms for each selected class.

If the school contact has not received the assessment material three days before the school’s first assessment date or if any forms are missing, please contact forms@cmec.ca immediately.

The school contact is responsible for the security and safe storage of the assessment forms at all times. The assessment materials must not be given to anyone except the test administrator on assessment day. They must not be duplicated or displayed in any way.

Once the school contact receives the material, they should:

  • print all forms;
  • check the list below to make sure all materials have been included and are correct; and
  • read through the information below to understand each form’s purpose.

School Questionnaire Login Form and Teacher Questionnaire Login Form

Principals and teachers of participating students will be provided with name specific login forms to access their respective questionnaire. See Step 7 for information on questionnaires.

School and Teacher Questionnaire Tracking Form

The school contact is asked to place a checkmark on the form to confirm that the teacher and school questionnaires were completed.

Student Tracking Form

The Student Tracking Form lists all students in the selected class and will be accessible to the test administrator digitally and as a paper form. The paper version of the form must be completed during the assessment session to record student participation and document any special circumstances. After the session, the test administrator will transfer the data from the paper form into the online Student Tracking Form. Once complete, the online Student Tracking Form will indicate whether a makeup session is required. Makeup sessions should be administered if fewer than 85 percent of the students in the selected class were present during the assessment session (see Complete Student Tracking Form). 

Student Login Form

Each participating student will receive a login form stating their name, CMEC ID, and password. Students will use these credentials to log in into PCAP. 

Assessment Script [MAILED]

To ensure that PCAP assessment sessions are conducted in the same way in all countries, the test administrator must follow the Assessment Script word-for-word.

The test administrator must be familiar with the Assessment Script before the assessment session.
