School Questionnaire

The school contact will receive the questionnaire login forms with the assessment materials.

Distribute the School Questionnaire Login Form to the school principal or designated staff as soon as the assessment materials arrive

The School Questionnaire Login Form includes login credentials and the link for accessing the School Questionnaire. Inform the school principal or designate that the questionnaire takes approximately 30 minutes to complete. It may be completed anytime but must be submitted no later than the date of the last PCAP session at the school.

Indicate that the School Questionnaire has been completed on the School and Teacher Questionnaire Tracking Form once the school principal or designate has informed the school contact of its completion.
