Step 2: Complete Student Identification Form
NOTE: THIS STEP DOES NOT APPLY TO SCHOOLS IN ONTARIO AND NOVA SCOTIA.
CMEC will randomly select one or more classes to be assessed at the school. Once this process has been completed, the provincial contact will inform the school contact which class or classes have been selected and will send them a Student Identification Form template. The school contact will be asked to complete one Student Identification Form for each selected class and return it to their provincial contact.
To help the school contact correctly complete the Student Identification Form, the tabs on the left-hand side provide further instructions on the form’s subsections.
