Step 5: Complete the Session Report Form

While administering the assessment, the test administrator should complete a Session Report Form. A new Session Report Form should be completed if a makeup session is held.

Page 1 of the Session Report Form should be completed with the test administrator’s name and position and the session number, date, and time; all questions on page 2 of the form should be answered.
