Home Questionnaire
The school contact must give each student their Home Questionnaire Login Form to take home to their parents/guardians as soon the assessment material is received. The Home Questionnaire is completed online using the link and login credentials found on the Login Form. The Home Questionnaire can be completed any time prior to the assessment but should be submitted no later than
assessment day.
The school contact may want to work with the teacher(s) of the selected class(es) to distribute the login forms. Each student has their own Home Questionnaire with their name and specific ID on it.
Students new to the class, whose names do not appear on the Student Tracking Form, should be given one of the three spare login forms. After doing so, record their name on the Student Tracking Form, on the row listing the same ID as on the login form. This ID will be specific to this new student. All TIMSS materials handed to them should list this specific ID, which is now linked to their name.
At the bottom of each Home Questionnaire Login Form there is a check box for parents/guardians to indicate that the questionnaire has been completed, as well as instructions for returning the Login Form to the school. The goal of this check box is to increase parent/guardian participation in the Home Questionnaire (i.e., this is not a permission form to participate in the test).
