Step 1: Return Forms to CMEC

The school contact is responsible for ensuring that all forms are completed accurately and submitted on time.

After the assessment administration is complete and all materials have been accounted for, the following forms must be completed and securely stored until CMEC confirms that all data have been verified:

  • Student Tracking Form: This form is completed online (and if needed, on paper).
  • School and Teacher Questionnaire Tracking Form: This form is completed on paper.

Once verification is complete:

  • If errors are found, CMEC will contact the school for clarification. Once any such errors are remedied, CMEC will instruct the school contact to shred all forms, including the login forms and the Assessment Script.
