Mailing Address

To provide the school with the Assessment Script or other required equipment (if applicable), CMEC requires the school’s mailing address.

Type in the school’s full mailing address. Please do not include a P.O. Box, as all materials will be delivered directly to the school.

Please also indicate whether Purolator services the school’s area. If Purolator does not service the school’s area, indicate which carrier should be used (e.g., Canada Post, private courier service).
