- School Questionnaire Login Form: For the school principal or designated staff
- Teacher Questionnaire Login Form: For the teacher(s) listed on the School and Teacher Questionnaire Tracking Form
- Home Questionnaire: For the parents/guardians of the students selected to write PIRLS
- School and Teacher Questionnaire Tracking Form: For the school contact to track school and teacher questionnaire completion
