Step 5: Receive Assessment Materials
At least five days prior to the first assessment date, the school contact will receive one email containing the electronic assessment forms for each selected class.
If the school contact has not received the assessment material three days before the school’s first assessment date or if any forms are missing, please contact forms@cmec.ca immediately.
The school contact is responsible for the security and safe storage of the assessment forms at all times. The assessment materials must not be given to anyone except the test administrator on assessment day. They must not be duplicated or displayed in any way.
Once the school contact receives the material, they should:
- print all forms;
- check the list below to make sure all materials have been included and are correct;
- read through the information below to understand each form’s purpose.
1. Student Tracking Form [Emailed]
The Student Tracking Form lists all students in the selected class. It will need to be completed by the test administrator during the assessment session. The test administrator will use this form to indicate student participation (see section Complete Student Tracking Form).
2. Student Participation Rates Form [Emailed]
The Student Participation Rates Form is used to calculate and document the student participation rate for each class to determine if a makeup session is necessary. Makeup sessions should be administered if less than 90 percent of the students in the selected class were present during the assessment session (see section Complete Student Participation Rates Form).
3. Session Report Form [Emailed]
On the Session Report Form, the test administrator will be asked to document the timing of the assessment session and answer a few questions on any special circumstances or problems that may have occurred during the assessment administration (see section Complete Session Report Form).
4. School and Teacher Questionnaire Tracking Form [Emailed]
The school contact is asked to place a checkmark on the form to confirm that the teacher and school questionnaires were completed.
5. School Questionnaire Login Form [Emailed]
Principals of participating schools will be provided with name specific login forms to access their respective questionnaire. See Step 6 for information on questionnaires.
6. Teacher Questionnaire Login Form(s) [Emailed]
Teachers of participating classes will be provided with name specific login forms to access their respective questionnaire.
7. Student Login Form [Emailed]
Each participating student will receive a login form stating their name, ID, and password. Students will use these credentials to log in to the test and the Student Questionnaire.
8a. Home Questionnaire Tracking Form [Mailed]
The school contact is asked to write the date the Home Questionnaire was sent out and the date the login slip was returned by parents to confirm that the Home Questionnaire was completed. If a login slip was not returned to the school, please leave the right column on the form blank.
8b. Home Questionnaire Login Form [Mailed]
Each participating student will receive a login form stating their name, and providing a ID and password for their parent/guardians. Parent/Guardians will use these credentials to log in to the Home Questionnaire.
9. Test Administrator Login Form [Emailed]
This form should ONLY be given to the test administrator. Each class will be provided one set of test credentials to preview the PIRLS Player, as well as the codes required to proceed through each part of the assessment. Please be aware these login credentials may only be used once.
Assessment Script [Mailed]
To ensure that PIRLS assessment sessions are conducted in the same way in all countries, the test administrator must follow the Assessment Script word-for-word. For ease of reference, the assessment script will be printed and mailed to all participating schools. The test administrator should become familiar with the Assessment Script in advance of their assessment session(s). A digital version of the Assessment Script is always available here.
